2010 APPLE POPCORN FESTIVAL
PRINCE AND PRINCESS CONTEST
SATURDAY, SEPTEMBER 18^TH

Contestants must be ages 4 to 7. Entries are limited to the first 15 boys and girls. The contest will be from 12:00 to 12:45 pm. We ask that all participants arrive no later than 11:45 am.

Children will be interviewed based on interests provided on this form and asked to perform a talent; however no talent is required to participate. Talent examples are singing, dancing, jokes, magic tricks, etc. If music is provided, the CD should be labeled with the childs name and track number. Talent should be limited to just a few minutes.

Please complete this entry form and return it to the address below no later than Monday, September 13th. A confirmation will be sent to all applicants confirming whether or not the child may participate in the contest based on limited number of entries. Please provide an email address below so we may send you a confirmation. There is no entry fee.

Each child will receive a gift bag for participating in the contest. If you have questions, please call 765-563-3135 and ask for Pat.
